Graduation (Master's Degree Completion) Requirements

1. Complete all coursework with a B- or above average.
2. Pass PLT exam. Submit passing score report to Education Department.
3. Complete graduation forms even if you are not planning to participate in the graduation ceremony. Forms located at http:/matrix-sws.newhaven.edu/student

Certification Requirements

1. Schedule exit interview with Ms. Phyllis Gwatkin, Certification Officer, at 203-932-7336 or at pgwatkin@newahven.edu.
2. Things to bring to the exit interview:

• ED 170 Application Form (still need link to this)
• Bank check or money order in the amount of ....?
• UNH Certification checklist (still need link to this)
• All state-required exam score reports
• Official undergraduate transcripts from all schools
